I. Introduction

You are reviewing and acknowledging this Telehealth Informed Consent because you are seeking healthcare services utilizing telehealth technologies facilitated through the Newman Clinic platform. This Telehealth Informed Consent supplements the Terms of Use, Privacy Policy, and Notice of Privacy Practices of Newman Clinic, rather than replacing them.

By creating an account, starting a consult, clicking “I consent to telehealth,” checking a related box, or using any other acceptance protocol presented through the Newman Clinic Platform, you indicate that you have reviewed and accepted the risks described herein of receiving services via telehealth technologies and consent to receiving services.

A record of this Telehealth Informed Consent is maintained by your Provider at Newman Clinic. Your ongoing participation in services through the Newman Clinic Platform serves as continued acknowledgment of this consent.

II. What is Telehealth?

Telehealth involves the delivery of health and wellness services using electronic communication, information technology, or other means between a licensed, certified, or registered healthcare professional at one location and a patient in another location. These services may include diagnosis, treatment, follow-up, and/or patient education via synchronous or asynchronous methods such as video/audio interactions or store-and-forward technology. This consent informs you about the treatment methods, risks,and limitations of utilizing telehealth.

 III. What are the Possible Benefits of Telehealth?

Benefits of telehealth include easier and more efficient access to health and wellness services. You can obtain these services at convenient times, avoiding the need for in-office appointments. Follow-up carecan also be managed via telehealth. For follow-up care or if you encounter any issues, contact us through the Newman Clinic Platform at Ask@NewmanClinic.com. 

IV. What are the Possible Risks of Telehealth?

Some clinical needs may not be appropriate for telehealth, and your Provider will determine if telehealth is suitable for your condition. Technology failures may delay or disrupt communication. If you experience a technical issue during a telehealth visit, please call our support center at Ask@NewmanClinic.com.  Certain diagnostic tests may contain defects that could affect the accuracy of results and, in turn, the quality of care you receive. Third-party laboratory services, which are required for some diagnostic tests, are not guaranteed to be fully accurate and may impact the diagnosis or treatment plan. 

IV. What are the Possible Risks of Telehealth?

V. Patient Acknowledgments

By accepting this Telehealth Informed Consent, you acknowledge that:
1. You have reviewed and understand the risks, limitations, and benefits of telehealth.
2. The telehealth services provided may involve risks to the privacy of your health information.
3. Your Provider may be a physician assistant, nurse practitioner, physician, or another licensed healthcare professional.
4. Other individuals may be present during your telehealth visit, such as those assisting with technology or providing language translation services, and you will be informed of their role.
5. Information you provide through telehealth services is considered accurate, true, and complete.
6. Certain services may be provided asynchronously via store-and-forward technology, and you consent to this mode of communication if your Provider deems it appropriate.
7. No guarantees of prescriptions or specific results are provided. The decision to prescribe medication or treatment is based on your Provider’s professional judgment.
8. Technical failures beyond the control of Newman Clinic or your Provider may occur, but you agree to hold Newman Clinic harmless for delays or information loss due to such failures.9. Diagnostic services offered via the Newman ClinicPlatform may have defects or limitations that could impact the quality or accuracy of your medical care.10. You understand that you may need to seek additional resources for other health concerns not addressed via telehealth.
11. Communication with your Provider may involve delays. It is your responsibility to regularly check the Newman Clinic Platform for messages.
12. You have the right to withdraw consent for telehealth services at any time without affecting future care or health benefits.
13. You have access to your medical records pertaining to telehealth services in accordance with applicable laws.
14. You may request a pharmacy or laboratory of your choice if certain services are coordinated through the platform.
